What is High-sensitivity C-reactive Protein?

Two people with the same cholesterol can face very different odds of a heart attack, and inflammation is one reason why. hs-CRP measures that inflammation, and it predicts cardiovascular risk even when a standard cholesterol panel looks clean. The 2026 AHA/ACC guidelines treat inflammation as part of heart-disease risk, not a separate question.

What is hs-CRP and why does it matter?

hs-CRP is a blood marker of inflammation, and inflammation drives how plaque forms, grows, and ruptures in your arteries. C-reactive protein rises whenever there’s inflammation in the body, and the high-sensitivity assay picks up the low levels that predict heart risk. Over 30 years of follow-up in the Women’s Health Study, higher hs-CRP tracked with more cardiovascular events, on a par with LDL cholesterol, so it captures risk a lipid panel alone can miss.

What is a good hs-CRP level?

A good hs-CRP is below 1 mg/L, which is low cardiovascular risk. 1 to 3 mg/L is moderate risk, and above 3 mg/L is high risk. One caveat matters: a single high reading often reflects a temporary cause like a cold, injury, vaccine, or dental work, not your baseline. Any value above 10 mg/L is almost always acute, so wait at least two weeks after an illness resolves and retest. Because hs-CRP can move a borderline estimate up or down, the 2026 AHA/ACC guidelines use it as a risk enhancer when your ten- or thirty-year risk sits near a decision threshold.

What is a normal hs-CRP by age?

In NHANES 2021-2023, the median hs-CRP was about 1.31 mg/L for adults aged 18-29 and 1.8 mg/L for adults aged 70 and older. The chart below shows the full distribution by age and sex.

hs-CRP vs CRP

hs-CRP and CRP measure the same protein (C-reactive protein), but hs-CRP uses a more sensitive assay that can detect much lower concentrations. Standard CRP tests are used to detect significant inflammation from infections or autoimmune conditions, while hs-CRP is specifically designed to measure the subtle, low-grade inflammation that predicts cardiovascular risk.

Does lowering hs-CRP reduce heart attacks?

Lowering inflammation appears to lower risk, not just the number. In the JUPITER trial, people with normal LDL but elevated hs-CRP who took a statin had significantly fewer heart attacks and strokes, and statins lower hs-CRP through a pathway separate from their effect on cholesterol. Diet, weight loss, regular exercise, and quitting smoking all lower hs-CRP too.
