Coverage policies checked September 28, 2026.
Most health insurance plans don’t cover the Galleri cancer screening test, but there are exceptions worth checking before paying out of pocket. TRICARE covers one test per lifetime for eligible beneficiaries, while PriorityMedicare Thrive lists a $150 copay once every other year. An employer may also pay for Galleri through a separate benefit, even when its medical plan doesn’t cover the test.
We reviewed published policies from Medicare, UnitedHealthcare, Aetna, Cigna, Anthem, and Arkansas Blue Cross and Blue Shield. Below, we’ll explain what those policies say, where coverage is available, and how to check the cost of both Galleri and any follow-up testing.
Does insurance cover Galleri?
UnitedHealthcare, Aetna, and Arkansas Blue Cross and Blue Shield explicitly name Galleri in policies that classify it as unproven, investigational, or otherwise not covered. Cigna’s policy limits covered liquid biopsies to specified cancer diagnosis and treatment uses; routine Galleri screening falls outside those criteria.
Galleri looks for cancer signals in blood. Insurance coverage for a blood test used to choose treatment for an existing cancer doesn’t establish coverage for screening someone without a cancer diagnosis.
| Insurer | What the policy says | Policy reviewed |
|---|---|---|
| UnitedHealthcare | Explicitly lists Galleri among multicancer screening tests considered unproven and not medically necessary. Applies to Commercial and Individual Exchange plans. | 2026T0588QQ, effective August 1, 2026 |
| Aetna | Explicitly lists Galleri as experimental, investigational, or unproven. | Tumor Markers, CPB 0352, reviewed August 25, 2026 |
| Cigna | Covers liquid biopsies in specified cancer treatment settings. Routine Galleri screening doesn’t meet those criteria. This is an interpretation of the criteria, not a named Galleri exclusion. | Policy 0520, effective September 13, 2026 |
| Anthem / Elevance-affiliated plans | The available older policy considers circulating tumor DNA panel testing outside listed indications not medically necessary. We couldn’t verify a current version. | GENE.00052, published December 28, 2023 |
| Arkansas Blue Cross and Blue Shield | Explicitly excludes multicancer early detection tests, including Galleri, under its coverage criteria. | Policy 2023030, reviewed July 2025 |
| Original Medicare | No routine Galleri screening coverage identified. A new federal coverage pathway begins no earlier than 2029, subject to further requirements. | CMS screening exclusions and 2026 law, §6221 |
These policies don’t describe every plan sold or administered by each insurer. Arkansas BCBS’s policy, for example, doesn’t apply to every Blue Cross and Blue Shield company. Your benefit document determines which rules apply to you.
Does Medicare cover Galleri?
Original Medicare doesn’t currently provide routine coverage for Galleri screening. Medicare Advantage plans can offer additional benefits: Priority Health’s 2026 PriorityMedicare Thrive plan lists Galleri with a $150 copay once every other year.
Congress has created a route to broader coverage. The law enacted February 3, 2026 allows a new multi-cancer early detection benefit beginning January 1, 2029. A test must have FDA authorization, and CMS must approve coverage through its national coverage determination process. The law also phases eligibility by age and limits testing frequency. It doesn’t guarantee Galleri coverage in 2029 or make every Medicare beneficiary eligible then.
Does TRICARE cover Galleri?
TRICARE covers Galleri for eligible beneficiaries 50 or older with an elevated cancer risk, with a limit of one test per lifetime. Its national coverage policy lists qualifying risks including documented hazardous exposures during military service, specified smoking or vaping history, HIV, a solid organ transplant, or cancer in two or more first-degree relatives.
Documented informed consent is required. Your provider must obtain prior authorization before ordering the test and drawing blood. Ask your TRICARE contractor about your eligibility and out-of-pocket costs before scheduling. A recommendation to repeat screening annually doesn’t change the lifetime coverage limit.
Can an employer pay for Galleri?
Some employers offer Galleri separately from medical insurance. Micron provides a useful example: its 2026 benefits guide describes a no-cost Galleri benefit for eligible employees, while stating that the test isn’t covered under its medical plans. Employees must use the designated Micron/Galleri ordering site. Ordering through another route can leave them responsible for the full price.
Some health plans also offer coverage directly. Curative announced Galleri coverage for eligible members, with $0 copay and deductible subject to its plan conditions. Check current eligibility and ordering instructions with your plan or benefits team.
How much does Galleri cost without insurance?
Empirical offers Galleri for $849 when paying out of pocket.
Galleri’s list price is $949. GRAIL says a reduced self-pay price of $799 or less is available through many healthcare providers. The price depends on who orders the test and which payment arrangement you use. Galleri pricing FAQ.
The $799 price is a provider self-pay example, not a price available through every ordering route. Blood collection is included when you use GRAIL’s lab partners. Source: GRAIL.
You may be able to use HSA or FSA funds. Confirm eligibility with your account administrator before paying. Those are your funds; HSA/FSA eligibility doesn’t mean your health insurer covers Galleri.
Does insurance cover follow-up tests after a positive Galleri result?
A positive Galleri result requires diagnostic evaluation. Coverage for the blood test doesn’t guarantee coverage for subsequent scans, specialist visits, or biopsies. GRAIL advises patients to work with their clinician and insurer to obtain authorization for diagnostic testing and offers patient advocacy support if coverage is denied. Galleri follow-up FAQ.
A cancer signal isn’t a diagnosis, and a negative result doesn’t rule out cancer. Galleri should be used alongside recommended screening, including mammograms and colorectal screening when appropriate. Our Galleri evidence review examines what the test detects and what follow-up can involve.
How to check your Galleri insurance coverage
Before ordering, ask your insurer or benefits team:
- Does my exact plan cover Galleri for my age and risk factors? Ask about a separate employer benefit, too.
- Which ordering provider and lab must I use? Confirm whether prior authorization is required.
- What will I pay, and how often is testing covered? A one-time benefit doesn’t pay for annual screening.
- How would diagnostic follow-up be covered if a signal is detected? Ask about network requirements, authorization, and cost-sharing.
Get the benefit details and any required authorization in writing before paying. If your plan excludes Galleri as investigational or unproven, a prescription alone won’t establish coverage.
